1. Privacy Notice

Who I am

Julie Butterworth is the data controller responsible for the personal information described in this notice.

Information I may collect

Depending on how you contact me and the services you use, I may collect:

·   identity and contact information, including your name, date of birth, address, telephone number, email address and emergency contact details;

·   appointment, payment and transaction information;

·   health and wellbeing information that you provide during enquiries, consultations or treatment, including symptoms, diagnoses, medication, treatment history and clinical notes;

·   communications between us, including emails, forms, messages and feedback;

·   technical information generated when you use the website, such as IP address, device and browser information, and cookie preferences; and

·   information necessary to meet legal, professional, safeguarding, insurance or regulatory responsibilities.

How I obtain your information

Most information is provided directly by you when you contact me, complete a form, book an appointment, make a payment or attend a consultation. With your knowledge or where otherwise permitted by law, I may also receive relevant information from a parent or guardian, another healthcare professional, a referrer, a payment provider or a booking provider.

Why I use your information and my lawful bases

I use personal information only where I have a lawful basis to do so. Depending on the circumstances, this may include:

·   Contract: to respond to an enquiry, arrange and provide appointments, take payment and administer the services you have requested.

·   Legitimate interests: to run and improve my practice, maintain appropriate records, communicate with you, manage complaints, protect the security of the website and establish or defend legal claims, where those interests are not overridden by your rights.

·   Legal obligation: where record-keeping, safeguarding, tax, insurance, court or regulatory requirements apply.

·   Consent: where I ask for a specific choice, for example to send optional marketing communications or to use a non-essential cookie. You may withdraw consent at any time, although this will not affect earlier lawful processing.

Health information is special-category personal data. Where I process it for the provision and management of your care, I rely on the condition for health or social care under Article 9(2)(h) UK GDPR where applicable and process it subject to professional confidentiality. In circumstances where that condition is not available, I will identify another valid condition, such as your explicit consent under Article 9(2)(a), before processing the information.

Clinical information and confidentiality

Clinical records are kept to support safe, appropriate and continuous care. They may include your health history, assessment information, treatment provided, advice given, outcomes and relevant correspondence. I treat clinical information as confidential and do not use it for unrelated purposes.

My services are complementary and do not replace medical diagnosis, prescribed treatment or emergency care. Where appropriate, I may encourage you to consult your GP or another regulated healthcare professional.

Who I may share information with

I do not sell personal information. I share it only where necessary and with appropriate safeguards. Recipients may include:

·   website hosting, secure email, appointment booking, clinical documentation, accounting and payment service providers;

·   professional advisers, insurers, accountants, IT support or legal advisers;

·   another healthcare professional or service, normally with your knowledge or consent;

·   a parent, guardian or person authorised to act for you; and

·   police, courts, safeguarding bodies, regulators or other authorities where disclosure is required or permitted by law, or where necessary to protect someone from serious harm.

Service providers act under contractual and confidentiality obligations and may process information only for the agreed purpose. Some providers may store or process information outside the UK. Where this occurs, I will use an appropriate UK data-transfer safeguard or rely on another lawful transfer mechanism.

How long I keep information

I retain information only for as long as it is needed for the purpose for which it was collected and to meet professional, legal, tax, safeguarding and insurance obligations. Different categories of information may have different retention periods. Clinical records are retained in line with applicable professional guidance and insurance requirements, after which they are securely deleted or destroyed. You may contact me for information about the retention period that applies to a particular record.

Security

I take reasonable technical and organisational measures to protect personal information against accidental loss, unauthorised access, alteration or disclosure. These measures include appropriate access controls, secure systems, confidentiality procedures and careful selection of service providers. No online system is completely risk-free, but suspected personal-data breaches are assessed and reported where required by law.

Your data-protection rights

Depending on the circumstances, you may have the right to:

·   ask for a copy of your personal information;

·   ask for inaccurate or incomplete information to be corrected;

·   ask for information to be erased or its use restricted;

·   object to processing based on legitimate interests or to direct marketing;

·   receive certain information in a portable format; and

·   withdraw consent where processing relies on consent.

Information, not individual medical advice

Website content is provided for general information and education. It is not a diagnosis, prescription or substitute for advice from your GP, consultant, pharmacist or another appropriately qualified healthcare professional. Do not disregard medical advice or delay seeking it because of something you have read here.

The website is not an emergency service. If you believe that you or someone else needs urgent medical help, contact NHS 111, your GP, or 999 in an emergency.

No guarantee of outcome

Health and treatment outcomes differ between individuals. Testimonials describe individual experiences and do not guarantee that another person will experience the same outcome. No website content or consultation creates a promise of a particular result.

Accuracy and availability

I take reasonable care to keep the website accurate and current, but information can change and I cannot guarantee that every item is complete, error-free or continuously available. I may suspend, withdraw or change website content without notice.

4. Booking and Cancellation Policy

Booking an appointment

Appointments may be requested through the booking system, by email, by telephone or by another method I make available. A booking is confirmed when you receive confirmation from me or from the booking system. Please ensure that your contact details and relevant information are accurate.

Fees and payment

The current fee will be shown at booking or confirmed before your appointment. Unless otherwise agreed, payment is due at or before the appointment. Card payments may be processed by a third-party payment provider. Package or prepayment terms, where offered, will be explained before purchase.

Cancellations and changes by you

Please give at least 48 hours’ notice if you need to cancel or rearrange an appointment. Cancellations made with less than 48 hours’ notice, and missed appointments, may be charged at the full appointment fee. I may waive or reduce the charge where there are exceptional circumstances, at my discretion.

If you arrive late, I may need to shorten the appointment to avoid delaying the next client. The full appointment fee may still apply.

Cancellations by me

Occasionally I may need to rearrange or cancel an appointment because of illness, emergency or circumstances beyond my reasonable control. I will give as much notice as reasonably possible and offer an alternative appointment or refund any fee paid for the cancelled appointment. I am not responsible for indirect costs such as travel, childcare or loss of earnings.

Suitability and clinical discretion

Before or during an appointment, I may decide that treatment should be modified, postponed or not provided where it would be inappropriate or unsafe, where essential information has not been provided, or where another form of assessment or care is more suitable. Where appropriate, I will explain the reason and suggest that you contact your GP or another healthcare professional.

Medication and existing healthcare

Do not stop or change prescribed medication or medical treatment without consulting the clinician responsible for it. My services are intended to work alongside, not replace, appropriate medical care. Support relating to cancer treatment, neurological conditions, metabolic health or GLP-1 medication is complementary and does not replace specialist, oncology, prescribing or emergency care.

Conduct and safety

I aim to provide a respectful and safe environment. I may end or decline an appointment where behaviour is abusive, discriminatory, threatening or otherwise makes safe treatment impossible. Fees already incurred may remain payable where permitted by law.

Complaints

If you are unhappy with any aspect of the service, please contact me promptly at clinic@juliebutterworth.co.uk so that I can try to resolve the matter. Information about relevant professional complaints procedures is available on request.

Your statutory rights

Nothing in this policy affects your statutory rights as a consumer.
